Emily suffered from a rare immune disorder.

Her pediatric records from Mercer’s home had been falsified, but hidden laboratory results showed progressive organ complications.

Sarah was the best possible donor because she was Emily’s identical twin.

James felt anger twist into disbelief.

“If they needed Sarah to save Emily, why not ask us?”

“Because asking would expose the kidnapping,” Marcus said.

Mercer had treated Emily privately for years. When her condition worsened, he needed Sarah.

Natalie discovered the twin partly because Mercer’s staff accessed Sarah’s medical records for compatibility testing.

Norma planned to commit Sarah to Hawthorne, where the procedure could occur under institutional consent.

The injuries complicated the plan.

That may have been why Sarah was abandoned rather than taken.

They could not admit her visibly injured without triggering scrutiny.

Instead, Mercer moved Emily and Natalie to the island while Norma’s family prepared another attempt at Sarah’s transfer from the hospital.

James asked the hospital to place Sarah under federal protection.

Sarah overheard enough to understand Emily was sick.

“Can I help her?”

James sat beside her.

“You never have to give anyone part of your body because they tell you to.”

“But she’s my sister.”

“Yes.”

“Will she die?”

James looked toward Dr. Walsh.

The doctor answered carefully.

“Not tonight. She needs treatment, but there may be other options.”

Sarah touched the bruises on her wrist.

“Grandma said if I didn’t help, it would be my fault.”

James closed his eyes.

The adults had already placed responsibility for Emily’s survival on an eight-year-old.

“No,” he said. “It is never your fault when adults refuse to ask safely.”

A Coast Guard team prepared to reach the island.

The property belonged to Mercer Health Foundation, but Frank’s boat had stopped transmitting near a marine exclusion zone.

Satellite images showed one main clinic, staff housing, and an old lighthouse.

A storm approached from the Atlantic.

The rescue window narrowed.

Chris insisted on joining the command boat.

A tunnel connected the clinic to the lighthouse foundation.

Agents entered and discovered a boat dock hidden inside the rock.

Frank’s vessel was there.

The captives were not.

A second boat had departed minutes earlier without a registered transponder.

Chris found blood near the dock.

Testing later showed it belonged to Frank.

Someone had injured him.

Perhaps Natalie fought back.

Perhaps Elise turned against Mercer.

Inside the lighthouse was a locked cabinet containing video messages.

One showed Emily addressing Sarah.

“I’m sorry they told me you stole my life.”

She appeared frightened but composed.

“Mom Natalie says you didn’t know about me. She says James is my dad too.”

James watched from the hospital.

Emily continued.

“Dr. Mercer says I’m sick because you got the strong blood. I don’t think that’s true anymore.”

The recording ended with Natalie entering the frame.

“James, Mercer has another donor.”

Her face was pale.

“He doesn’t need Sarah now. He needs both girls for something else.”

She held up a medical form.

The procedure was not simple marrow collection.

Mercer planned to perform an experimental immune-cell exchange using both twins to create a patented therapy.

Sarah and Emily were research subjects.

The foundation could earn billions.

Their reunion had been delayed until the girls were old enough for the trial.

James’s employer.

His company had funded Mercer’s research and arranged his conference trip.

James called his supervisor.

No answer.

His employee portal had been disabled.

A new message appeared on his phone.

The sender attached a photograph.

Natalie and Emily sat inside a small aircraft.

Elise stood behind them.
