No sabotage.

An electrical fault in a warming unit overheated grease residue inside ventilation ducting.
Maintenance inspections had missed deteriorating insulation.
The suppression system worked.
Staff responded reasonably.
One employee suffered minor smoke inhalation and a superficial burn.
No catastrophe.
But our assessment found something useful.
During evacuation, several recruits waited for direct instructor orders instead of using the nearest marked exit.
Why?
Because training culture had emphasized obedience so strongly that initiative under obvious emergency conditions had weakened.
