Chapter 1 - The Authorization at the Intake DeskEmily West’s scream cut through the ambulance bay as the paramedics rolled her toward the emergency entrance.

She was thirty-four weeks pregnant.

One hand gripped the yellow stretcher rail while the other pressed against the underside of her belly. The contractions had begun less than an hour earlier, violent and impossibly close together.

The ambulance doors had barely closed behind them when David West charged across the pavement.

He did not ask whether the baby was alive.

He did not ask why Emily was bleeding.

He seized her forearm and dragged her halfway off the stretcher.

“She apologizes to my son before anyone treats her!”

Emily cried out as the safety strap cut across her advanced pregnancy.

The female paramedic struck David’s hand away while her partner caught Emily’s shoulders before she hit the ground.

“Back away!”

David tried to grab her again.

The paramedics secured Emily against the stretcher and rushed her through the sliding doors.

Michael entered behind his father.

He wore the same expensive navy coat he had been wearing when Emily collapsed at home. There was no snow on his shoulders, though he claimed he had followed the ambulance immediately.

Emily looked directly at him.

He did not look frightened.

He looked inconvenienced.

A nurse named Hannah Cole hurried from the emergency intake desk.

Emily pointed toward both men.

“I do not consent to either man speaking for me.”

Hannah’s expression sharpened.

“You heard her,” she told the clerk. “Flag the chart for patient-only authorization.”

Michael pushed past a security officer and slapped his palm against the intake counter.

“I’m her sole medical decision-maker. It’s already in her chart.”

The clerk opened Emily’s patient portal.

A document appeared beneath the emergency tab.

Durable Medical Authorization—Spousal Control During Maternal Crisis.

It bore Emily’s electronic signature.

David’s name appeared on the witness line.

Emily stared at it.

“I never signed that.”

Michael leaned toward the computer.

“She doesn’t remember half of what she signs.”

Hannah printed the document.

The signature bent unnaturally downward through the final letter of Emily’s surname. Emily always finished her name with a sharp upward stroke.

Hannah placed the page inside a clear evidence sleeve.

“Security, neither man goes beyond the secured doors.”

David’s confidence weakened.

Michael’s did not.

“Open the rest of the file,” he ordered.

Hannah ignored him and checked the portal’s audit history.

The authorization had been uploaded six weeks earlier from an administrative account belonging to Westbridge Medical Foundation.

A second attachment had been uploaded at the same time.

Its title was hidden behind an encryption symbol.

Emily gripped the stretcher rail as another contraction tore through her.

Hannah turned the monitor so she could see it.

“Emily, do you want us to open the hidden attachment?”

“Yes.”

Michael lunged across the counter.

“Don’t let her read it!”

The security officer caught him around the chest and forced him backward.

David said nothing.

His face had gone completely still.

The cursor hovered over the attachment.

The loading bar began moving.

Then the filename appeared:

MATERNAL INCAPACITY, NEONATAL RELEASE, AND ASSET CONTINUITY PLAN.

Emily looked at her husband.
